Here you can see the CD Storage inserts in action. Each CD-1030 Storage Insert holds 4 disks and 4 index cards. Note the Disk number corresponds to the number on the index card, making it easy to re-file the disk after you use it.

In this photograph you can see the index card template, for an area or team archive. These templates are easy to create in any word processor.
